Status Disclosure Among Black Gay and Bisexual Men Living with HIV in Baton Rouge, LA
In Baton Rouge, Louisiana, Black gay and bisexual men (BGBM) living with HIV exist in the context of economic inequality; hegemonic HIV-stigma, racism, and homophobia; as well as a lack of HIV-education in their families and communities. In addition, the legal and policy contexts, of both the city and state, are particularly hostile to gay and bisexual men. Consequently, for BGBM, disclosure of an HIV diagnosis comes with the potential of significant social risks, including potential disruption to social and familial support networks, as well as loss of housing and economic security. In addition, nondisclosure means that men risk facing social or criminal sanctions. Little research has examined how structural inequalities and social location shape the disclosure process, and the benefits and consequences of disclosure. Employing a structural and intersectional analysis, this study explored: the disclosure process and experiences of Black gay and bisexual men in Baton Rouge; how intersecting stigmas and oppression constitute the disclosure process and its outcomes; and the structural and intersectional outcomes of disclosure. A sociological understanding of HIV disclosure can shift public health approaches to HIV among BGBM toward seeing disclosure in its full social and relational context, and away from viewing it simply as a responsible risk-reduction strategy. Public health efforts should intervene on structures, not on individual behavior, as privileging disclosure purely from an HIV prevention standpoint places blame and responsibility on those living with HIV, and ignores the barriers created by intersectional oppressions.
